I found my equipment, what's next?
Select the “I’d Like to Request This Item” button and complete the intake form in the name of the person receiving the device. Once the form is submitted, the item is removed from the inventory and a unique Request ID number is displayed. Save this number in case you need to call us about your request. Once your request is approved, you will receive next steps via email or snail mail. If we have questions, we will follow up by phone or email.
